I am aware of the dangers related to volunteering for this event on this particular date. I promise not to hold the company, its officers, agents, or employees liable or pursue legal action against them. This waiver absolves the company of any duty for injuries sustained on the premises before to, during, or after the activity.


By signing this agreement, I agree to hold the Covenant Purpose and Restoration Family Center completely harmless, including financial responsibility for any injuries sustained, regardless of the cause or circumstances.


I will do all in my power to obey corporate staff and all safety requirements, and I will seek clarification if necessary.


I authorize Covenant Purpose and Restoration Family Center to distribute and reproduce the materials for the following purposes: Portfolio showcase, advertising, marketing, branding, educational, digital promotions, internet videos, online courses, media, other commercial or non-commercial purposes.


I agree to allow Covenant Purpose and Restoration Family Center, Inc. to take or capture my photos and videos.

Volunteer hours are generally available weekly on Thursdays and Fridays from 9:30AM-12:00PM.  We will announce special volunteer opportunities as they become available.
